Acceptance of Terms
By accessing or using the services provided by Clarity & Co. Counseling, you agree to be bound by these Terms of Service. If you do not agree with any part of these terms, you must not use our services.

Services Provided
Clarity & Co. Counseling offers mental health counseling services. Our services are intended for individuals seeking therapeutic support. 

Client Responsibilities
Clients are expected to provide accurate information regarding their mental health history and to communicate openly with their therapist. Clients are also responsible for attending scheduled appointments and providing notice for cancellations.

Fees and Payment
All fees for services rendered will be communicated prior to the start of therapy. 

Confidentiality
Your privacy is important to us. We will maintain the confidentiality of your personal information and records in accordance with state and federal laws, including the Health Insurance Portability and Accountability Act (HIPAA).

Limitation of Liability
Clarity & Co. Counseling shall not be liable for any indirect, incidental, or consequential damages arising from the use or inability to use our services.

Governing Law
These Terms of Service shall be governed by the laws of the State of New Hampshire and the State of Tennessee.

Changes to This Privacy Policy
We may update this Privacy Policy from time to time. Any changes will be posted on this page with an updated effective date.

Good Faith Estimate:
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
